phone is locked - need a code

My daughter accidentally locked her phone, but when we hit "unlock", it prompts for a Security Code (4 numbers). She doesn't know what this is (and doesn't think she entered one before). How can we unlock the phone (LG 500G)?

I've used Tracfones since 2001, and have NEVER had this problem of PUK codes. I just purchased the new LG800G phone - even had it rushed to me over night because it is my ONLY phone. Now, I'm being asked for a PUK code (it is the only screen I have). I cannot make or receive any calls except 911.

How do I fix this problem immediately - or I will be returning this phone back to Tracfone and never using the service again. I have had enough.
