Trying to get lost minutes back after getting a new phone

Can someone please help me get my minutes back that i've lost when upgrading to my new phone? It's been since August 16th that my new phone was activated, but my minutes didn't transfer. I had at least 3000 test's and at least 2000 minutes and 2.44 gigs of data. I was told the morning someone activated the phone for me that they would transfer my minutes, but this never happened.

I had to purchase a new card at that time so I was able to use the phone.
