LG 840 No service

LG 840 No service

My LG 840 worked great for 2 weeks and then showed No Service on screeen. I can't send or receive calls/texts. I also noticed a new icon on the third screen SIM App. I looks like it requires a code - but I don't have one. Any ideas??

Janet, I tried to respond, but got this message:
